SERVICES

Enrollment

Requirements:

  • SF9 (Report Card) (photocopy and original)

  • Photocopy of PSA Birth certificate

Process:

  1. Fill-up Learners Enrollment Survey Form

  2. Submit to grade level enrollment group

  3. Proceed to the Guidance Office for interview

  4. Proceed to the administration office for sectioning.

Request for SF10/Form 137

For transfer to other school (Basic Education)

  • Please ask your adviser or registrar to enroll you in the DepEd Learners Information System. The school will send the SF10 directly to the school if already tagged in LIS.

For college students (Higher Education)

  • Send/submit request letter from your school indicating your name and last school year attended at DYCNHS.

For employment and other purposes

  • Fill-up request form (face to face) or send/submit request letter stating your purpose (online). Please indicate the school year last attended.

  • If filed by representative (not required if parent), please send an authorization letter with your contact number, photocopy of your ID and your representative for verification.

Request for Certificate of Enrollment

  • Face to Face: Fill-up request form and submit to the registrar. (Form is available in the front of the Administration Office)

  • Online: Send request letter stating your purpose, current grade level and section and contact number. (Please email to 301627@deped.gov.ph)

  • If filed by representative (not required if parent), please send an authorization letter with your contact number, photocopy of your ID and your representative for verification.

Request for Diploma/Certificate of Good Moral/Other documents

  • Face to Face: Fill-up request form and submit to the registrar. (Form is available in the front of the Administration Office)

  • Online: Send request letter stating your purpose, last school year attended in DYCNHS and grade/section and your contact number. (Please email to 301627@deped.gov.ph)

  • If filed by representative (not required if parent), please send an authorization letter with your contact number, photocopy of your ID and your representative for verification.

Request for Second Copy of Diploma

Requirements

  • Affidavit of loss or Certification from Barangay Captain (if loss due natural calamity)

  • Letter of request for reconstituted copy of diploma, please state your last school year attended.

If filed by representative (not required if parent), please present an authorization letter with your contact details for verification.

This will take 1 to 4 weeks process because the documents will be sent to the Division Office for verification and signature.

Certification, Authentication and Verification (CAV)

Requirements for CAV

  1. Diploma (Original) with (2) Certified True Copy

  2. Transcript of Records/ Form 137-E with (2) Certified True Copy

  3. Certification of Graduation from School

  4. Certification of Graduation from DepEd Division Office

  5. Certification of School Last Attended (if undergraduate)

  6. List of Graduates (Duly Certified Correct by Authorized Officials)

  7. Latest I.D. Picture 2 x 2 (2 copies)

  8. Birth Certificate Original-NSO (with Photo Copy)

If filed by Representative / Proxy:

  • Authorization Letter (Special power of Attorney)

  • ID of applicant and proxy with Xerox copy
